The completion of the form below is an essential piece of documentation which allows the tracking of each staff member's completed trainings.

How does our program gather family data to individualize programming?*

- A food intolerance causes indigestion, a food allergy causes hives
- A food intolerance is being a picky eater, a food allergy may cause sneezing
- A food intolerance is a reaction to food that may cause discomfort and special accommodations, a food allergy is potentially serious, life threatening condition, and often requires an immediate response
How does our program's No Outside Food Policy keep children safe?*
